This study aims to construct a conceptual model of differentiated learning integrated with digital-based assessment that is relevant for mathematics teachers in the era of digital education transformation. The research is grounded in the need for mathematics teachers to implement adaptive learning that accommodates students’ diverse characteristics while utilizing digital technology as an effective and efficient assessment tool. The research employed a mixed-methods approach, integrating both qualitative and quantitative methods. The qualitative phase involved observations, interviews, and document analysis to explore teachers’ needs and perceptions regarding digital assessment and differentiated instruction. Meanwhile, the quantitative phase was conducted to test the validity and reliability of the instruments and to analyze data using descriptive statistical techniques. The findings indicate that the application of digital-based assessment enhances the effectiveness of differentiated learning by providing real-time feedback, increasing students’ learning motivation, and facilitating teachers in analyzing learning outcomes. Furthermore, the study developed a preliminary design of a differentiated learning model with digital assessment comprising four main components: (1) learning design based on students’ needs, (2) the use of interactive digital assessment tools, (3) data-driven learning adjustment, and (4) integration of continuous feedback. This study is expected to contribute to improving mathematics teachers’ competencies in implementing adaptive learning aligned with the principles of the Kurikulum Merdeka and the demands of 21st-century education.
